All patients receiving hemodialysis require a permanent method of connection to remove toxins from their bodies. For those receiving hemodialysis, a graft or fistula, which join an artery and vein together, provides access for blood to flow to a hemodialyzer, where toxic waste is removed.

Even when patients meticulously care for their grafts and fistulas, clotting or infection can occur. And these situations often require immediate attention to avoid a serious health crisis and to ensure patients can continue with their regularly-scheduled dialysis. Signs of a problem at the access points include swelling, redness, a sensation of warmth, and pain.

When fistulas and grafts become clotted and blood flow becomes restricted, our expert team of interventional radiologists performs minimally-invasive procedures to improve or restore blood flow.

WHAT IS THE ACCESS MANAGEMENT PROGRAM?

 It is a educational program on the care and maintenance of your access. We provide you with education and inspection techniques that will help you maintain the access health.

WHY IS ACCESS MANAGEMENT IMPORTANT?

Access Management will make you feel better! Every access must be kept healthy with good blood flow to ensure you maximize the benefits of your dialysis treatments.

The Access Management Program includes:

  • Education on access types and options by our nursing staff and physicians
  • Regularly scheduled office visits with our physicians
  • Active ultrasound surveillance plan throughout the year specifically designed for you
  • Assisting you with ongoing care and maintenance, and review of the do’s and don’ts of access care

Fistulogram

This diagnostic procedure uses a form of real time x-ray called fluoroscopy, which helps monitor the function of a dialysis graft or fistula. This procedure consists of an interventional radiologists injecting a dye in the patient, which allows them to carefully examine the flow of blood from the arterial anastomosis to the central circulation system. This is very effective in identifying any problems in the vascular system that are associated with the fistula or graft. This procedure may be an option for patients that have prolonged bleeding, cannulation difficulties, weak thrill or bruit, high venous pressures and/or low flow rates.

Sometimes, a doppler study is needed to help a physician evaluate the blood flow of a patient’s veins and arteries, prior to the placement of a fistula. This test is able to depict the blood going through your blood vessels, using ultrasound waves. This helps the physician to create a “road map” of your arteries and veins.

During this procedure, the doctor can also treat the catheter if it gets blocked due to blood or fibrin deposits. Oftentimes, medication is used in conjunction to help dissolve the blood or fibrin. Alternatively, the clot can be removed mechanically.
